McDonald's is actually quite scrupulous about that -- your average burger gotten at the counter is cooked and assembled very quickly by a set of teenagers who would really rather be somewhere else, but for the commercials, McDonald's actually has "food stylists" who create the sandwiches seen in their advertising (plus they use a few tricks like piling all the toppings along the edge facing the camera). They actually made a video a few years ago to address this (very common) observation and were very forthright about what's done:
