No, we never defined how long "on notice" lasts.  Off the top of my head and in the absence of a consensus, I would think that three years is probably well past its expiration date.  Six months maybe?

In light of the fact this is a long time editor who decided to breach our trust, specifically the privilege of not needing to be review patrolled, and considering this isn't the first time we've had to call them on the carpet, I lean towards the harshest possible interpretation of what it means to be "On Notice". Specifically, they have six months to keep their nose clean, and failure to do so means we will escalate our response accordingly.
