To start the process, type "/respec" (without the quotes) and hit enter.
You'll be in a screen very similar to the one in the character creation process and/or level up screen where you choose powers.
Remember - you're doing this sequentially from level 1 up. So it's like you're doing the level up procedure all over again! It's a good idea to take notes about when you took certain powers and when you added slots to them. Keep careful track of levels as you go. It's easy to lose your place and forget where you are. As an extreme example, you could make the mistake of choosing your travel power LAST. Which would mean that every time you exemplered down below your current level you'd lose the travel power, and not just at below level 14.
There are a couple of different free downloads of programs that allow you to plan out your leveling, too. Can't remember where they are right now and don't have the time to look. Maybe someone else has that info.
